I've attached a sample order form as well as a document featuring the products I’ll offer when I take your photos on Picture Day. I prefer parents use the order envelopes I provide (payments can be inserted inside). I do understand there are times when people need to print the order form in advance. Payment is due when photos are taken. I accept cash, check, PayPal, Venmo. If you use an app to send your payment, please make sure you add your team name and swimmer's name in the "Note" section. My PayPal and Venmo information is noted on the top of the attached order form. I’ll have a QR Code available for these apps when we meet.
